Prosthodontic vouchers are valid for 60 days
Please ensure your patient's voucher is started before 30 days to allow treatment to be processed in DOVE after the expiration.  
If it is to exceed 60 days, please submit a treatment plan to Oral Health for approval.

Emergency Vouchers;
•	Emergency vouchers are for one emergency care only. 
•	Only send a treatment plan if it pertains to the tooth treated on that same day (i.e. root canal treatment).

REMINDER: If you have entered a filling item code twice for the same tooth, 
please insert tooth surface to allow us to process the voucher.

REMINDER: Please upload all Treatment Plans to DOVE.

Please ensure patient is signing the voucher as well as the dentist.
